This worksheet will help you to keep track of your progress and to remind you of Departmental and Graduate School requirements.
As part of the university's evaluation of graduate programs, each Ph.D. student in Entomology will be subject to Graduate Outcome Assessments (GOAs) during their graduate program. The data gathered will provide critical feedback regarding the effectiveness of the graduate program and better inform the student and advisor on student progress toward his/her degree. The assessment will occur at 3 programmatic milestones (qualify / candidacy exam, dissertation exit seminar, and dissertation defense), a yearly assessment of professional and disciplinary benchmarks (progress report), and 2-years post-graduation (post-survey). Go to "Doctoral Graduate Outcome Assessment Forms" for Ph.D. students for a description of Graduate Outcomes Assessment and the necessary forms.
Progress reports are due annually. Please submit completed progress reports before the announced deadlines. Progress report forms for other programs, such as BEES, MEES, and MOCB are acceptable if you are in one of those programs.
Ph.D students should use this form during Entomology qualifying exams. Following the exam, please return the completed form to the Graduate Program Assistant (email@example.com). Also, after passing your exam, fill out the Application for Admission to Candidacy form found on the graduate school web site.
